A Learning Collaborative Summit
Tuesday, May 14, 2019: 8:30 a.m. - 4:00 p.m.
Wednesday, May 15, 2019: 8:30 a.m. - 4:00 p.m.
Check-In & Breakfast: 8:00 a.m.
Program begins at 8:50 a.m.
We strive for collaborative learning focused on expertise, tools and resources which are organized and deployed in a manner to promote strong collaborative learning and sharing.
OBJECTIVES:
- Identify best practice care models through presentations and collaborative interactions with other providers
- Incorporate various process improvement tools in your projects to enhance outcomes and share your successes
- Create opportunities for networking amongst providers and community providers.

EVENT INFORMATION:

PARKING: Parking is free and located directly behind the Hurst Conference Center in the tower parking garage.
- MEALS: The collaborative will be providing a continental breakfast of pastries and fruit during check-in, lunch, and snacks. If you require a vegetarian meal please note that on your registration.
- BEVERAGES: The collaborative will provide coffee, tea, and water.
- HOTEL INFORMATION: Will share information at later date.
